This full-time role offers the unique opportunity to deliver patient-focused care directly within the comfort of patients' homes in Sacramento. We are seeking a compassionate and skilled Physical Therapist who excels at personalizing treatment plans and fostering a supportive environment for patients recovering from illness, injury, or surgery.
In this position, you will:
- Conduct comprehensive physical assessments and review medical histories.
- Create individualized treatment plans tailored to each patient's specific needs and goals.
- Implement therapeutic exercises and training programs to enhance patient recovery.
- Educate patients and their families on injury prevention, wellness, and the recovery process.
- Monitor patient progress and document rehabilitation milestones accurately.
- Collaborate with a multidisciplinary team to ensure integrated patient care.
- Maintain meticulous patient records in compliance with healthcare regulations.
Qualifications:
- Master's degree in Physical Therapy.
- A minimum of one year of experience in home health as a Physical Therapist.
- Current licensure to practice as a Physical Therapist in the state of operation.
- Valid CPR certification is required.
- Licensed driver with the ability to travel to various business locations.
