Role and Responsibilities:

  • The Clinical Manager supervises apotentiallyinterdisciplinary team of nurses,therapistsand social workers,to assureappropriate clinicalservicesareprovided by the Agency.This person, or similar alternate, is available at all times during operational hours and participates in activities with respect to professional services. Collaborates between departments tofacilitate and assure the delivery of high-quality patient care.

Essential Functions:

  • Supervises the day-to-day clinical operations (including ensuring workflow tasks are completed even during staff absences) and all required patient care is completed including,but not limitedto:
  • Review and follow-up on complaint forms and all QI reports.
  • Onsite supervision ofclinicians as assigned.
  • Oversee the instruction of theclinicalteam inprovidingquality service andmaintainingstandards, including in-home visits to direct and evaluate patient care.
  • Proactively address operational and clinical practices to prevent patient harm and/or organizational deficiencies or inefficiencies.
  • Ensure that the patients plan of care is executed as written and reassessed by theappropriate healthcare professional when there is a significant health status change in the clients condition, at the physicians request,and after hospital discharge.
  • Recommend policies, procedures and/or protocols based upon industry regulations and evidence-based clinical practice standards.
  • Assure that staff perform according to Agency standards as well as legal/regulatory standards.
  • Conduct case conferences (informal,supervisoryor interdisciplinary) to discuss client issues, plan of care,and discharge planning,including documentation of such conferences via the Case Conference Coordination Note in the medical record.
  • With theassistanceof the Clinical Field Staff Supervisor and Patient Scheduling Coordinator, oversee the delegation of assignments to staff according to licensure, training, anddemonstratedskills, as well as the clinical needs of the patient.
  • In collaboration with the HR Department and Regional Director,participatein recruitment of qualified personnel and implementation of effective strategies to promote staff retention at acceptable parametersestablishedby the Agency.
  • Ensureclinicalstaffmembersmaintainup-to-date credentials and adheres to job responsibilities and productivity requirements by conductingtimely, thorough performance reviews annually,andprovidingmore frequent feedback on low performance as needed.
  • Assistin determiningappropriate staffinglevels andmaintainadequate staffinglevelsto meet patientrequirements.
  • Approve weekly schedules and ensure productivity standards are met.
  • Review and approve bi-weekly payroll.
  • Recommend and collaborate on in-service education programs to promote staff development.
  • Take administrative on-call, whenrequired.
  • Maintain nursing knowledge base to be current with accepted community standards and best practices.
  • Participate in community awarenessprogramsas necessary.
  • Understand and effectively interpret operational budgets andrespondto negative variances.
  • Create strategies to meet corporate goals,including teamobjectivesthat are attainable. Monitor and evaluates effectiveness using measurable criteria.
  • Provision of monthly staff meetings to meet agency communication plan.
  • Adhere to andparticipatein Agencys mandatory HIPAA/Privacy Program and Employee Compliance Program.
  • Read and adhereto all Agency policies and procedures and follow the Employee Handbook Guidelines.

Education Requirements :

  • Bachelors Degree in nursingrequired.

Qualifications and Requirements:

  • CHHA nursing experience at the Case Mgr. Level.
  • Minimum of one - two years experience in home health care.
  • Current, valid RN license issued by NYS Department of Education required.
  • Medicare home health experience and management experience.
  • Organized, detail-oriented,andpossesseseffective communication skills both orally and in writing.
  • Understands the delivery of home health care andis able toproblem-solve effectively.
  • Capable of prioritizing and handling multiple tasks of critical importance simultaneously.
  • Functionswell in an atmosphere of change and interruption.
  • Effective management skills.
  • Mustpossessa valid NYS drivers license and automobile liability insurance.
  • Must have dependable transportation andableto drive in all types of weather.

Work Environment:

  • TheClinical Manageris primarily in an office setting and may be exposed to outdoor conditions.

The working conditions are classified as light work:

  • Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of forcefrequentlyor constantly to lift, carry, push, pull or otherwise move objects, including the human body. Light work involves sitting most of the time.
