Job Description:

  • Develop and obtain approval for treatment plans from referring physician.
  • Treat patients according to physician-prescribed plans.
  • Assist nursing department in Restorative Aide training.
  • Supervise Physical Therapy Assistants in patient care, adhering to state regulations.
  • Communicate patient progress and plans with team.
  • Attend Patient Care Conferences and Rehabilitation Conferences as needed.
  • Participate in staff in-services training.
  • Document evaluations, progress notes, and recertification in compliance with PT Board regulations.
  • Record treatment changes following policies.
  • Provide family/caregiver training for discharge.
  • Assist in discharge planning.
  • Recommend and facilitate ordering of durable medical equipment for patient mobility.
  • Report equipment issues promptly.
  • Foster positive relationships with patients, families, and colleagues.
