A qualified professional who provides physical therapy services to patients as prescribed by the attending physician under the direct (but not on-site) supervision of a licensed Physical Therapist.
Responsibilities:
- Treat patients to relieve pain and/or develop or restore function to reach the highest level of independence possible.
- Instruct patients and their families in in transfer techniques, home exercise program and activities of daily living, use of prosthetic, orthotic and assistive devices
- Write clinical progress notes on the date of the visit for inclusion into the patient’s permanent record.
- Perform an on-site, joint visit with the assigned Physical Therapist initially, and communicate regularly for instruction in changes in the patient’s program.
- Communicate regularly with the case manager and other health care professionals assigned to a case.
- Graduate of an accredited Physical Therapist Assistant program.
- Licensed by and currently registered with the New York State Education Department.
- One year of practical experience as a licensed physical therapist assistant.
